Someone shot today would likely be shot with a modern weapon. Not an antique. So it's modern medicine versus modern weapons. Even if survivable, that doesn't automatically mean no permanent damage and disability. Just not dying doesn't mean he could still perform his job as president. The congresswoman that got shot not too long ago survived but had to resign as she couldn't do the job any more.

I don't know if we can really compare Lincoln's injuries to Gabrielle Giffords'. According to an FBI analysis of the gun believed to have been used by Booth (they authenticated it), it was a .44. The news reports I've been able to Google say Giffords was shot with a 9mm. (Here's that analysis, if anyone's interested: www.fbi.gov/about-us/lab/forensic-science-communications/fsc/jan2001/schehl.htm/ ) Maybe a modern assassin wouldn't use a percussion-cap weapon, but a single-shot .44 is possible ... and possibly easier to smuggle past security than a larger handgun.

At point blank range, that's all they'd need. And, it's small.
From what I've read, the number one fear of the Secret Service is that the assassin won't care about getting away. Then, all they need is to get close enough to kill.

Important to note that the .44 we're talking about isn't the same as the Remington .44 Magnum, or the .44 Special, but rather a cap and ball round the happens to measure .44 inches.
